MADISON - A Wisconsin investigation has determined there was nothing mechanically wrong with a free fall ride when a 12-year-old Florida girl fell about 100-feet to the ground.
The state Department of Commerce report released Thursday indicates the operator at the top of the Terminal Velocity did not follow protocol when Teagan Marti fell July 30 at the Extreme World amusement park in Wisconsin Dells.
The report says Marti was dropped before the cage in which she was riding reached the top and before a net was in proper position.
Thirty-three-year-old Charles Carnell was the dive master and was charged with one count of first-degree-reckless injury.
Marti was severely injured and may be paralyzed. She was transferred Wednesday to a Florida hospital. The ride remains closed.

TOWN OF CONOVER - A local veteran says he feels West's family's pain.
Harold Kressin of Conover is an Army veteran.
Kressin says it's hard to hear about soldiers losing their lives overseas, and it's especially hard when they're from your community.
Harold Kressin, Army Veteran, says, "Everytime a man gets killed or a woman gets killed in service, it's just heart-breaking to me, I cry. Because these people are not going to have the freedoms that we have."
TOWN OF CONOVER - Fire destroys a garage in Vilas County.
It happened in the Town of Conover at 3551 North Country Lane.
The Conover Volunteer Fire Department responded to the call around four Wednesday afternoon after the owner noticed fire around his feet while welding in the garage and called 911.
When firefighters arrived the garage was fully engulfed.

CHICAGO, IL - A meteorite that fell in Wisconsin earlier this year now has a name: Mifflin.
The Field Museum in Chicago said Wednesday that the Meteoritical Society named the meteorite after Mifflin Township, where it fell on April 14. The group also has classified it as an ordinary chondrite meteorite.
Pieces of the meteorite are on display at the Chicago museum.
